In 2014, the Kane County MAPP Executive Committee embarked on a journey founded in its vision & values to develop a Community Health Assessment (CHA) using the Mobilizing for Action through Planning and Partnerships (MAPP) process. Results from four assessments were analyzed to inform the identification of prevailing strategic issues. These priority issues were determined to be Behavioral Health, Chronic Disease, and Income and Education. Goals and action plans were developed for each priority issue. These action plans are being implemented and aligned to improve the health and well-being of the Kane County community.
Forces of Change Assessment
The FOCA is aimed at identifying forces – such as trends, factors, or events – that are or will be influencing the health and quality of life of the community and the work of the local public health system.
Community Health Needs Assessment
The CHNA is a systematic, data-driven approach to determining the health status, behaviors and needs of residents in Kane County, Illinois.
Local Public Health System Assessment
The LPHSA is used to understand the overall strengths and weaknesses of the public health system based on the 10 Essential Public Health Services.
Community Themes and Strengths Assessment
The CTSA is conducted to form an understanding of community issues and concerns and perceptions of quality of life across the state.
