Community Health Assessment
Kane County is partnering with Conduent Healthy Communities Institute (HCI) to conduct the 2020 Community Health Assessment. HCI currently supports Kane Health Counts, which features more than 250 health, social, and economic indicators for Kane County. HCI will build upon insights leveraged from HCI’s data platform to conduct a comprehensive assessment process of the county through:
- Thorough analysis that triangulates secondary health, demographic and socioeconomic data, and community input/primary data to understand the greatest needs and opportunities for improvement in the community
- Community engagement to understand the greatest health needs –an approach that focuses on community input research and incorporating community voices, including special attention to vulnerable populations
- Guidance on how best to prioritize needs in order to have the greatest impact
HCI will conduct various assessments to inform the Kane County Health Department community health assessment (CHA) meeting Illinois Department of Public Health Certified Local Health Department code and local hospital community health needs assessments (CHNAs) meeting IRS Section 501(r) Final Regulations.
